Welcome to West Park Dental’s “Whitening for Life” reward program. We are proud to have you has a patient in our office and are thrilled for you to be able to take advantage of this fantastic program. This program is a reward program for our patients who are diligent with their continuing care.

Our "Whitening for a Lifetime" program is easy and is as follows:

1. If you have purchased bleaching from West Park Dental in the last 2 years you are grandfathered in. Congratulations!
2. If you are new to the program the next time you are in for your professional cleaning and exam we will take impressions for custom fitted whitening trays. It will take approximately 1 week to fabricate the trays.
3. Once trays are completed, detailed instructions will be given on how to apply the whitening gel properly. You will receive a whitening kit with your trays.
4. The initial cost of the bleaching is being offered at a Special Introductory Price of $199.
5. Future appointments must be made at a minimum 6 month interval from the initial date for regular cleaning and exams to maintain dental health. At your recare appointment you will receive one tube of bleach-FREE!
6. If the 6 month appointment must be cancelled for any circumstance, it must be reschedule within 2-4 weeks.
7. If the recall visit is completely missed and not rescheduled within the allotted time, the "Whitening for a Lifetime" reward program ends and bleach will need to be purchased at the regular price of $13 per tube.
